Patient Terms and Conditions

Last updated: 12 Oct 2025

1. Introduction

Welcome to Docy.

Docy is a digital platform that enables medical practices ("Practices") to manage patient information, documents, and communications securely online.

These Patient Terms of Use ("Terms") govern your access to and use of Docy as a patient ("Patient"). By creating or accessing your patient profile on Docy, you agree to these Terms.

If you do not agree, you should not access or use Docy.

2. Our Role

Docy (the "Platform") is provided by Docy ("we", "us", or "our").

Docy is not a healthcare provider. We do not offer medical advice, diagnosis, or treatment.

We provide technology that allows healthcare professionals to collect, store, and share patient information securely and efficiently.

Your healthcare relationship remains directly with your Practice or healthcare provider, not with Docy.

3. Patient Accounts

3.1. Account Creation

You may receive an invitation from a Practice to create a patient account on Docy. When you accept this invitation, a Patient profile is created, which may include information provided by both you and your healthcare provider.

3.2. Accuracy of Information

You are responsible for ensuring that all personal or medical information you submit through Docy is accurate, up to date, and not misleading.

3.3. Security

You must keep your login credentials secure and not share them with anyone else. If you believe your account has been compromised, please notify us or your Practice immediately.

4. How Your Information is Managed

4.1. Control of Data

Your personal and medical data may be added or updated by:

  • You (the Patient), and/or
  • Your Practice or its authorised users (e.g. doctors, nurses, administrators).

In some cases, your Practice will remain the data controller of information they add, meaning they decide how and when it is used within the scope of your medical care.

4.2. Documentation of Consultations

Your Practice may document consultations and interactions in a variety of ways to ensure accurate clinical records and continuity of care. These may include, but are not limited to:

  • Written notes;
  • Digital forms or attachments;
  • Audio recordings of consultations;
  • Photographs, images, or scans relevant to your care.
  • etc.

By using Docy, you acknowledge and consent that your Practice may create and store such documentation as part of your medical record.

4.3. Sharing with Other Healthcare Professionals

To support coordinated care, your Practice may choose to share parts of your record, including consultation documentation, with:

  • Other doctors, specialists, or healthcare facilities involved in your treatment; or
  • Substitute or locum practitioners assisting your Practice.

This sharing is done only as permitted by applicable law and your Practice's own privacy and consent procedures.

4.4. Access by You

You may access and review your data through Docy at any time. You may also request a copy of your data or request that it be shared with another healthcare provider, subject to legal and clinical requirements.

5. Data Portability and Continuity of Care

You may choose to share your medical information with another Practice on Docy.

When you do so, your records may be made available to that new Practice for ongoing treatment and care.

Once shared, that Practice becomes independently responsible for the data they receive.

6. Acceptable Use

You agree not to:

  • Upload false, harmful, or illegal content;
  • Access or attempt to access another person's data without permission;
  • Use the Platform for any purpose other than legitimate healthcare or personal recordkeeping;
  • Interfere with or disrupt Docy's operations or security.

We reserve the right to suspend or restrict your access if your account is used in a way that violates these Terms or applicable law.

7. Communications

You may receive secure messages, requests, or notifications from your Practice through Docy.

Docy may also send you system-related or security notifications by email or within the app.

We will never send marketing or promotional content without your consent.

8. Your Rights

Depending on applicable law, you may have the right to:

  • Access your personal information;
  • Request correction or deletion of incorrect information;
  • Withdraw consent for data processing (subject to medical recordkeeping laws);
  • Request that your data be transferred to another healthcare provider.

For such requests, please contact your Practice directly, or Docy if your account is under your own control.

9. Limitation of Liability

Docy provides the Platform "as is" and makes no representations about its suitability for any specific medical purpose.

To the maximum extent permitted by law:

  • Docy is not responsible for the accuracy, completeness, or medical appropriateness of any information uploaded by a Practice or Patient;
  • Docy is not liable for medical outcomes, advice, or diagnoses made by your healthcare provider using the Platform;
  • Docy's total liability to you is limited to the extent required by applicable law.

10. Termination

You may stop using Docy at any time by notifying your Practice or deleting your account (where this option is available).

Your Practice may also deactivate your access if your care relationship with them ends.

Docy may suspend or terminate your account if it is misused or required by law.

11. Updates to These Terms

We may update these Terms periodically to reflect legal, technical, or operational changes.

When material changes are made, you will be notified via the email associated with your account or when you next log in.

Continued use of Docy after such updates constitutes acceptance of the revised Terms.
